Maternal, Adolescent & Child Health Institute NPC, (MatCH) is a non-profit organization supporting public sector health services. MatCH implements donor funded projects to improve HIV and TB outcomes through providing technical assistance and direct service delivery support. MatCH is looking for suitably qualified candidates to fill the following positions under a project aimed at Achieving and Sustaining HIV/TB Epidemic Control in the Eastern Cape Province – Integrated Service Delivery Response.

Location: Amathole District , Eastern Cape ( Allocation of supported Sub districts within Amathole District will be confirmed at interviews )

Main purpose of the job To lead and support implementation of OVC Family Strengthening interventions. Support supervision of OVC Community Care Workers/ CCW’s in ensuring correct and eligible beneficiaries are enrolled into the program through, screening and assessment. Contribute to the OVC clinical care clinical care, and achievement of 95-95-95 targets, through mentoring of staff and implementation of OVC clinical strategies within the allocated districts. Monitor daily beneficiary serving in the community, ensuring appropriate quality differentiated services rendered are age appropriate, inclusive of caregiver participation and guide progression for successful program graduation. Work with relevant stakeholders for effective bidirectional referrals and case management.

Key performance areas Supportive supervision of OVC Community Care Workers (CCW’s) implementing OVC comprehensive services. Support and management of incoming referrals through agreed bidirectional referrals protocols from DREAMS primary services to OVC CCM Family Strengthening interventions for children and youth aged 0 – 17 years. Ensure program team enrol appropriate program beneficiaries to OVC CCM FSP, beneficiaries with elevated risks and vulnerabilities including those affected and living with HIV. Provide chronic disease and HIV/ TB management and health promotion for the OVC beneficiaries and ensure that clinical services are aligned to the SAG policies and HIV/TB guidelines. Ensure CCW’s offer comprehensive differentiated care package of services to beneficiaries aligned with 4 core program domains: Health, Schooled, Safe and Stable. Ensuring quality services rendered in line with beneficiary needs and vulnerabilities assessed. Work with partners linkage team and health facility point of contact (POC) to ensure eligible beneficiaries are received. Assign and monitor daily and weekly caseloads to CCW’s.Support beneficiaries with clinic appointments as necessary, e.g. Plans for treatment pickups, fast track consultation on review dates especially for treatment failing patients. Monitor correct use, completion of OVC CCM case management and data collection tools. Support supervision, monitoring of field staff (household visits and V3 group session sit ins) and ensure proper data recording on tools and registers for daily, weekly and monthly reporting of CCM services progression. Monitor referrals and ensure successful and complete referrals to other services within the community, partners and or at health facility, e.g. PrEP, SRH, HTS/HSS, ART, GBV/VAC, VMMC, etc. Monitor quality of CCM and V3 interventions (EBI) implementation and completion. Attend health facility, community and stakeholder’s relevant meetings for continuous support and successful implementation of OVC CCM FS activities. Collect completed beneficiary data from CCW’s, review and submit for capturing and filling. Ensure all registers are completed appropriately and filed accordingly. Contribute to successful case identification, management and assist in writing/ reporting best practices.

Required minimum education, training, and technical support. Relevant Nursing Diploma NIMART certificate TB certificate

Professional body registration Registration with the Nursing Council of South Africa.

Minimum work experience At least (2) years of experience of working in the areas of HIV, TB and community programs. Good knowledge of comprehensive HIV treatment and care. Experience of 3 years group facilitation

Desirable additional education, work experience and personal abilities Knowledge of HIV, TB and DREAMS and differentiated care programs. Knowledge of case management Knowledge of HIV treatment A relevant degree in social services/ health is desirable. Excellent time management skills Supervision and good group management skills Good written and verbal communication skills Ability to manage and resolve conflict. Facilitation skills and able to work independently. Report writing and interpersonal skills. Strong administrative and planning skills Ability to work independently and as part of a team.Proficiency in English and isiXhosa Professionalism, customer centered. Ability to work under pressure and in a diverse environment. Ability to work independently in a resource limited setting, as well as in a team environment. Attention to detail. Self-motivation with high regard for excellence, work ethic, values, and integrity Available to commence work immediately .
